PRIMARY CARE

 Primary Care is available at a flat rate of $25.00 per visit. Primary care includes general medicine, family medicine, pediatrics, and internal medicine. Services includes the treatment of acute, chronic, and preventive care.

SPECIALTY CARE

Choose from over 200 specialty providers at significant discounts up to 60% from usual and customary rate. CareNet continues to expand its specialty network. Please check the web site for updates at www.mycarenet.com.

URGENT CARE

All Urgent Care visits will be provided at a flat rate of $50. Other services may be required at the time of the Urgent Care visit. You will be advised of the cost of any service needed in advance and be given the option to accept it or not. Urgent Care services are only covered at CareNet participating centers; please refer to the list at the end of the handbook.

PRESCRIPTION DRUG MEDICATIONS

Substantial discounts are available for prescription medication CareNet written by one of its Primary Care Provider. You will also have access to prescription medication not included on the formulary if carried by a Network pharmacy. CareNet pharmacies prices are substantially less than retail pharmacies. See list of Network Pharmacies in the handbook.

DENTAL SERVICES

As a member of CareNet you have access to our select Network of Dental Providers. CareNet offers a Dental Exam at flat rate of $25. CareNet also offers two simple cleanings per year at a flat rate of $25 per cleaning. Prior to scheduling a dental cleaning, other services may be required like the Dental Exam. Other dental procedures are available at substantial discounts. Refer to your dental provider, the web site or the member services department for available services.

VISION SERVICES

You can take advantage on exceptional discounts on eye exams, eyeglasses, contact lenses, and laser vision correction procedures through Vision Select at 1-800-641-3809 or call the member services department at 1-877-251-4005. The glasses package includes 50% off frames and lenses.

BEHAVIORAL HEALTH

 Inpatient and outpatient services are available at substantial discounts for evaluation and treatment of mental health disorders and substance abuse. Up to 50% discount is available for these services.

 LAB

 Lab tests starting at $5.00 per test from an approved list of procedures are available at the providers office. This rate represents a savings of up to 60% of usual and customary charges.

DIAGNOSTIC TESTS

CareNet offers invasive and non-invasive radiology procedures up to 50% discount from usual and customary rates.
