HOW DOES YOUR MEMBERSHIP CARD WORK ?
- ELIGIBILITY - Eligibility is established upon receipt of payment. Payments must be received by the 10 th of the month prior to the month of coverage. Once you receive your membership card, you can start receiving discounts on health services. In order to maintain your active membership, your monthly payments must be received by the 10 th of each month. We encourage timely payments to avoid loss of membership.
- PROVIDER SELECTION - Our providers speak English, Spanish, Creole, French, and many others. In addition we have services for the hearing impaired. Make an appointment directly with the provider of your choice, please refer to the list at the end of this handbook. If you are unable to keep your appointment, notify the provider immediately and re-schedule to a more convenient date and time. You can change your appointment by calling at least 48 hours in advance. Always check the web site to obtain the most current provider directory or call member services at 1-877-251-4005.
- ACCESS - It's Easy!! On the day of your appointment, present your card and pay for your discounted services. There are no limits on the number of times you can see your provider. It is as easy as making an appointment. For more information call the member services department Monday through
